Before You Register

We just need to confirm a few details about your requisition and coverage.

💡 You can still continue even if you don’t have a FlowLabs requisition or an Ontario health card.

SECTION A – Requisition & Code

⚠️ A doctor’s requisition is required to register. Please contact your doctor and return once you have it. Exit Registration

Look for a paper or email titled Laboratory Requisition or Test Order Form.

It lists your name, doctor, and ordered tests. If you have this form, select “Yes.”

Is your requisition from FlowLabs?

(You don’t need a FlowLabs requisition to register — we accept all lab requisitions.)

Yes — my form says FlowLabs

No — it’s from another lab (e.g., LifeLabs, Dynacare)

I’m not sure — help me check

💡 We accept requisitions from all labs. You do not need a FlowLabs requisition to continue.

Look near the top of your requisition. If you see the FlowLabs logo (like the example below), select Yes.

If it lists another lab, select No.

Either way, you can continue registering.

Do you see a requisition code on your FlowLabs requisition?

It’s a short mix of letters and numbers near the top-right corner (e.g., FL-12345678).
Enter it if you can see it — otherwise select “No.”

Look at the top-right corner. You might see something like FL123456.

If you still can’t find it, choose “No.”
