Sperm DNA Fragmentation Index (DFI) Test in Ontario

A sperm DNA fragmentation test (DFI test) is an advanced semen evaluation that can provide deeper insight into male fertility beyond a standard semen analysis. It helps identify DNA damage that could impact embryo development and pregnancy outcomes (even when fertilization is not impacted). 

This test is often recommended along with semen analysis to provide the most comprehensive evaluation of sperm health. A normal semen analysis test result does not mean that you will have a normal DFI test result. All testing is performed in our specialized lab led by experts in male reproductive health.

When This Test is Recommended

You may be advised to complete a sperm DNA fragmentation test if:

  • You are planning on having children now or in the future
  • A semen analysis appears normal, but you continue to experience fertility challenges
  • You have varicocele
  • You have experienced recurrent pregnancy loss
  • You are undergoing or planning assisted reproductive treatments (e.g., IUI or IVF)
  • Your clinician recommends further investigation into sperm DNA integrity

What This Test Evaluates

A sperm DNA fragmentation test assesses the percentage of sperm in a sample with damaged or fragmented DNA, referred to as the DNA fragmentation index (DFI). A high DFI level is linked to reduced fertility, lower success rates with assisted reproduction techniques (such as IUI and IVF) and a high risk of miscarriage.

Understanding Your Results

Your results will indicate the level of DNA fragmentation in your sperm. Higher levels of fragmentation can affect fertilization and pregnancy outcomes. Based on these findings, your clinician may recommend lifestyle changes, additional testing, or targeted treatment options.
